GOING HOME

Discharge instructions, dictated by your physician, will be given to you by your nurse. Some patients will need additional treatment or care following discharge. St. John’s Mercy has a variety of services available:

Home Health Services
St. John's Mercy Home Care provides intermittent skilled nursing and therapies in a patient’s home. Services include nursing, physical, speech and occupational therapies. Medical social workers and home health aides can provide services in the home as well. St. John's Mercy Home Care offers specially trained nurses in the areas of orthopedic, cardiopulmonary, intravenous therapy and wound care. St. John's Mercy Home Care is a Missouri state-licensed, Medicare-certified and JCAHO accredited home health agency. Services are provided in the following areas: St. Louis city and county and the counties of Jefferson, Franklin, St. Charles, Lincoln, Warren and Gasconade. Home Infusion Therapy
Often patients are discharged to their homes with intravenous therapy ordered by their physician for treatments including antibiotic therapy, pain management, total parenteral nutrition, chemotherapy and hydration. St. John's Mercy Home Infusion Therapy offers specially trained home health nurses and pharmacists to care for patients needing these services at home. Emergency Response System
The Emergency Response System allows people the freedom to live independently at home knowing that in an emergency help is available at the press of a button, 24 hours a day, 365 days a year. The Emergency Response System connects the subscriber to assistance when needed. Call 636-239-8044 for more information.

Hospice Care
St. John's Mercy Hospice provides supportive care for the physical, emotional and spiritual needs of patients with terminal illness. Hospice is a philosophy based on the belief that people with advanced illness can live and die among friends and family in familiar surroundings. A team of nurses, social workers, home health aides and volunteers provide care for the patient and family members. Family support and bereavement services are an integral part of the hospice experience.

Expert pain management and symptom control allow patients to live as fully as possible, remaining alert with minimal pain so that their last days may be spent with dignity at home or in a homelike setting. For details, call 636-239-7128.

David C. Pratt Cancer Center
The David C. Pratt Cancer Center at St. John's Mercy Medical Center offers education and information on risk education, early detection, treatment and ongoing support for cancer patients and their families. Services include support groups, educational presentations and clinical trials through the St. Louis-Cape Girardeau CCOP (Community Clinical Oncology Program). Integrative services and support therapies including physical therapy, occupational therapy, speech therapy, acupuncture, massage therapy and wellness classes are available on site. Specialized clinical programs addressing incontinence and lymphodema are offered. For information on these programs, call 314-251-6571. The Cancer Center also includes the John F. Fertility Care Services
Couples learn to interpret the natural changes of a woman’s body to avoid or achieve pregnancy. An introductory session in the CREIGHTON MODEL FertilityCare™ System is followed by private sessions. For more information, call the hospital’s Fertility Care Services at 636-239-8595.

Dietitian Services
An outpatient appointment can be made with a registered dietitian on all aspects of nutritional care. A diabetes self-management training program is instructed by a dietitian (certified in diabetes education) and nurse educator. Other community programs are offered throughout the year. Rehabilitation Services
Outpatient therapy may be prescribed following acute hospitalization. St. John’s Mercy Sports and Therapy Centers provide treatment for individuals with a variety of neuromuscular and musculoskeletal diagnoses, including back/neck/limb injuries, sports injuries, brain injuries, spinal cord injuries, strokes, amputation, etc. The Center provides outpatient services including physical therapy, occupational therapy, speech therapy, pediatric services as well as cardio-pulmonary rehabilitation.
